Kefir After 70: Why I’m Adding Fermented Milk to My Daily Shake

The short version: Kefir is a fermented milk drink made with a mix of live bacteria and yeasts. I’ve added half a cup of plain kefir to my daily protein shake. In a Stanford study, people who ate more fermented foods every day for 10 weeks ended up with a more diverse gut microbiome and lower levels of 19 inflammation markers. The research on kefir itself is promising but still thin, and I’ll say where it stops.

I already put a lot into the shake Kathy blends for us every day. Fifteen ingredients, a full blender pitcher. What it did not have was anything alive.

Now it does. Half a cup of plain kefir goes in every day, the first step in the fermented foods experiment I started in Starting Fermented Foods at 71.

Here is what kefir is, why fermentation matters, and what the research actually supports.

What Is Kefir?

Kefir is a fermented milk drink that tastes like a tangy, drinkable yogurt.

It is made by adding kefir “grains” to milk. They are not grains at all. They are small, cauliflower-looking clumps of bacteria and yeasts living together, and they ferment the milk over about a day. The drink comes from the Caucasus Mountains, where people have made it for centuries.

What sets kefir apart from yogurt is the variety. Most yogurt is made with two or three bacterial strains. Kefir carries a mix of bacteria and yeasts, and store-bought brands commonly list around a dozen live cultures on the label.

What Does Fermentation Actually Do?

Fermentation is microbes eating the sugars in a food and turning them into something else. In kefir, they turn most of the milk sugar (lactose) into lactic acid, which is what gives it the sour taste and thickens it.

That one process does several useful things at once:

  • It pre-digests the food. The microbes break down lactose and some of the milk proteins before you ever drink it.
  • It adds live microbes. If the product is not heated afterward, you are drinking the bacteria and yeasts themselves.
  • It creates new compounds. Fermentation produces organic acids, some B vitamins and small protein fragments called bioactive peptides that were not in the plain milk.
  • It preserves the food. The acid keeps harmful bacteria from taking over. That is why people fermented food long before refrigerators existed.

Why Fermented Foods Matter for Longevity

The best study on this came out of Stanford in 2021, published in the journal Cell. Researchers took 36 healthy adults and split them into two groups for 10 weeks. One group worked up to about six servings of fermented foods a day, including yogurt, kefir, fermented cottage cheese, kimchi and other fermented vegetables, vegetable brine drinks and kombucha. The other group roughly doubled their fiber.

The fermented food group ended up with a more diverse gut microbiome, and it stayed more diverse for weeks after the study ended. Nineteen inflammation markers in their blood went down, including interleukin-6, a marker linked to the chronic low-grade inflammation that drives so much of aging. The high-fiber group did not see that drop in inflammation. (Wastyk et al., Cell, 2021)

Two honest caveats. It was 36 people, and the study’s main immune measure did not change in either group. But a diet change that lowers inflammation markers without a pill is exactly the kind of thing I look for, and it is the reason I eat kimchi too.

What the Research on Kefir Itself Shows

Less than the internet claims.

A 2023 systematic review in Nutrition Reviews looked at 16 randomized trials of kefir in people. It found kefir may help with oral bacteria linked to cavities and as an add-on during treatment for the stomach bacteria H. pylori. It found the evidence for cholesterol and blood pressure very limited, and rated most of the studies at high risk of bias. Its conclusion was that kefir looks safe for healthy people, and that better trials are needed before anyone makes firm recommendations. (Nutrition Reviews, 2023)

One finding is solid and useful for people my age. In an Ohio State study of 15 adults with lactose intolerance, kefir produced far less of the gas that signals undigested lactose than plain milk did, and cut flatulence by more than half. Because the microbes eat most of the lactose, a lot of people who cannot handle milk do fine with kefir. (Hertzler and Clancy, Journal of the American Dietetic Association, 2003) Worth knowing: that study was funded by a kefir company.

So I am not taking kefir because a trial proved it adds years. I am taking it because fermented foods as a group have real evidence behind them, kefir is one of the most diverse fermented foods you can buy, and the downside is half a cup of milk.

How I Am Adding It

Half a cup of plain kefir, poured into the blender with everything else in our daily longevity shake, every day.

A few things I learned before buying:

  • Buy plain and unsweetened. Flavored kefir can carry as much added sugar as a dessert. The shake already has berries for sweetness.
  • Look for “live and active cultures” on the label. That is what you are paying for.
  • Keep it cold, and never heat it. Blending does not hurt the microbes. Heat kills them, so kefir does not belong in hot coffee or anything cooked.
  • Start smaller if you are new to fermented foods. Some people get a little gas or bloating the first week or two while their gut adjusts.

Half a cup also adds a few grams of protein, which never hurts at 71.

What I Will Be Watching

I am not expecting to feel a dramatic change, and I would be suspicious of myself if I did. What I will watch is the same thing I watch for everything new: how my stomach handles it, my morning numbers in my health app, and the inflammation markers on my next blood panel. I will report back either way.

If you want the rest of what I do, my full daily regimen is in my protocols, the training side is under fitness, and every number I track is in tests and stats.

Live long and prosper,
