Creatine for Brain Health After 70: Why I Put 15 Grams in My Coffee

Creatine monohydrate was the first supplement I ever took. Somebody recommended it to me a year ago when I started this longevity journey, and it has been in my coffee every morning since.

Most people file creatine under bodybuilding. Powder in a shaker bottle, twenty-two-year-old trying to add fifteen pounds to his bench press. That is not why I take it, and at over 70 it is probably not why you would either.

Why Would Someone Over 70 Take Creatine?

Two reasons: muscle and brain.

I am not trying to bulk up. I am trying to keep what I have. Muscle loss after 60 is one of the quiet things that decides whether you are independent at 85, and creatine combined with resistance training is one of the few supplements with real evidence behind it for holding onto lean mass.

The brain is the part most people have never heard about, and it is the reason my dose is what it is.

Why Is 5 Grams Not Enough for Brain Health?

Because your muscles take creatine up easily and your brain does not.

Five grams a day will saturate muscle tissue. That is the standard dose and it has been the standard dose for thirty years. But the brain sits behind the blood-brain barrier, and creatine crosses it poorly. Researchers looking at cognition have had to use much larger amounts to move brain creatine levels at all.

Your brain is about two percent of your body weight and burns something close to twenty percent of your resting energy. It is the most energy-hungry tissue you own. As you age you produce that energy less efficiently, which is the whole argument for topping up the fuel supply.

What Does the Research Actually Say?

It says higher doses can raise brain creatine and may help cognition, but the evidence is younger and thinner than the muscle evidence.

Now the honest part. A 2024 systematic review looked across this literature and concluded the research does not yet support the theoretical case for creatine improving cognition. Several studies show effects only in vegetarians, whose baseline creatine is lower to begin with.

So this is not settled science. I take 15 grams because it sits between the 10 grams used in the newer cognition trials and the 20 grams used in the brain-uptake work, and because creatine has one of the longest safety records of anything on my shelf. That is a judgment call, not a conclusion.

Why Put Creatine in Coffee?

Because it dissolves better in hot liquid, and because I was already drinking the coffee.

I drink four cups every morning and I put a different supplement in each one. It is the simplest habit system I have ever used. The cup is the reminder.

Creatine has famously bad mixability, and its solubility goes up with temperature, so hot coffee genuinely does dissolve it better than a cold glass of water. I still use a frother, because it will settle at the bottom of the mug otherwise and you will end up drinking half your dose.

One myth worth putting down: people used to say caffeine cancels out creatine. That came from a small study in the 1990s, and later reviews have not held it up. Taking them together is fine.

I also add a pinch of salt. Creatine uptake does depend on a sodium-linked transporter, so the idea is not made up, but I have never seen a study showing that salt in your coffee improves absorption. Call that one my habit rather than my evidence.

What I Actually Do Every Morning

  • 15 grams of creatine monohydrate
  • Into one of my four cups of hot coffee
  • A pinch of salt
  • Frothed until it is fully mixed, then drunk before it settles
  • Every single day for a year, no cycling, no loading phase

Which Type of Creatine Should You Buy?

Monohydrate. It is not close.

There are half a dozen other forms sold — hydrochloride, buffered, ethyl ester, magnesium chelate, nitrate. None of them has beaten monohydrate in a head-to-head trial, and ethyl ester actually performed worse. Monohydrate has hundreds of studies behind it and costs the least per gram. Micronized monohydrate is the same molecule ground finer so it mixes more easily, which is worth the small premium if you are putting it in a drink.

Should You Take 15 Grams Too?

Probably not on day one.

Start at 5 grams and see how your stomach handles it. The most common complaint with creatine is bloating or GI upset, and it shows up more at higher doses. If you go up, go up gradually and split it across the day rather than taking it all at once.

Creatine also pulls water into muscle tissue, so drink more than you think you need. And if you have any kidney issue at all, this is a conversation to have with your doctor before you start, not after. Most of the long-term safety data is at 3 to 5 grams a day. Fifteen is well within what has been studied short-term, but I am not going to pretend there are decades of data at that dose.

Five grams will not bulk you up and it will not hurt you. It may help you keep the muscle you already have, which at our age is the whole game.
